When our residents have specific questions or concerns regarding dietary needs based on a current medical diagnosis or condition, our dietitian is readily available to assist them.

Working towards your best recovery

Las Colinas Post Acute offers a number of services to our patients. Our team of amazing specialists work together to create a targeted and individualized plan to help you regain as much independence as possible.

24-Hour Skilled Nursing

We are proud to offer qualified, experienced nursing care to our residents whether they are here for a short-term or long-term stay. We have Registered Nurses (RNs), Licensed Vocational Nurses (LVNs), as well as Certified Nurse Assistants (CNAs) available 24 hours a day to ensure that our resident’s skilled nursing needs are met.

OT – Occupational Therapy

Our occupational therapy team works hand-in-hand with physicians and surgeons to ensure that our patients reach their highest level of independence. Occupational therapists (OTs) work with patients who suffer from physically, mentally, and/or emotionally disabling conditions by developing an individualized care plan that improve basic motor functions and reasoning abilities. OTs provide customized interventions to improve individuals ability by providing treatment programs that may include: range of motion and strengthening exercises, functional ergonomics, and/or use of modalities to decrease pain and swelling. Occupational therapy focuses on the upper extremities to help our residents improve their ability to manage day-to-day living.

Post- Stroke Rehabilitation

Our experienced team of physical, occupational and speech therapists work closely with individuals who are recovering from stroke. While rehabilitation does not “cure” or reverse the brain damage that may have occurred from a stroke, rehabilitation is vital in reducing further brain injury and maximizing patient recovery. With motivation and social support from our staff, the primary goal of stroke recovery is to prevent secondary health complications, minimizing impairments, and achieving functional goals that promote independence.

Wound Care

We are proud to offer extensive wound care by our team of Board-Certified Wound Surgeons and treatment nurses who specialize in all types of wounds including: pressure, diabetic, vascular, traumatic, and post-operative. Our wound care team is also capable of replacing gastric and suprapubic tubes in order to further reduce unnecessary transfers. We have two specialized wound care nurses who work conduct weekly wound care rounds for patient consultation and follow up to ensure that wounds are properly cared for and promote maximum healing potential. While our surgeons are also available to our residents and family members for emergency consultations outside of regular rounds, our wound care surgeons regularly round several times per week. In addition to the surgeons that service Las Colinas, our facility employs two full-time wound treatment nurses.

PT – Physical Therapy

Las Colinas has an excellent physical therapy team that treats health related conditions, illnesses, or injuries that limit patients’ abilities to move and perform functional activities of daily living. Physical Therapy includes muscle strengthening and retraining, transfer techniques, stair climbing, training in the use of prosthetics, gait training, and adaptive equipment training. Physical therapists (PTs) use an individual’s history and physical examination to establish a care plan by setting goals that will be specifically tailored to each individual. Their plan may include pain management, exercises, stretching, manual therapy, education and other interventions. PTs work with individuals by maintaining, restoring movement and functional ability back to the patient, and preventing further loss of mobility before it occurs. PTs will help build strength, endurance, coordination, balance, and/or flexibility. Our ultimate goal is to restore maximal functional independence to each individual.
